G. A. Patrick

Today is going to be stressful for Jaiden. He is watching the children today while Peyton has rounds at the hospital. The routine seems to be their weekend mantra, so to speak, they do what they do from Friday through Sunday. And that they are on-call all weekend, that hasn't even been mentioned.

Jaiden met Peyton at the hospital as he was beginning his rounds, and Peyton was ending his. Since they see the same patients plus the new ones that come in, they always sit down and go over medical charts. "Jaiden, Damian Stevens has been having problems with depression, and I think he is going to need some medication to deal with it. Talk to him and see what you think; I can prescribe the medication tomorrow if you prefer, or if you think it will help, you can prescribe it tonight. Something to sleep might do the trick." "I'll take some time to talk with him and see if he will open up to me and then go from there.

"Thanks, Jaiden, I'll talk to you later this evening. I will go to the house and take care of the boys and give Dale and Charley a break. I'm so tired; I hope I don't fall asleep before the boys go to bed." Peyton kissed Jaiden and wished him a good evening. He would be at the hospital until at least midnight. The thought that he and Peyton would be able to skip a year of residency, thanks to their experience as Physician's Assistants, was beginning to sound particularly good to both.

As soon as Jaiden had time to look at Damian's chart, he went to Damian's room to speak with him about his case and see what he thought his patient would need to escape his depression.

"Good evening, Mr. Stevens, how are you feeling tonight?" "Jaiden, as far as pain, it is tolerable, and Dr. Brannen tells me the healing process is going well. The only problem I have is wondering what I will do once I get out of this hospital. I know I have to be careful about what I do, and I know there isn't going to be any motorcycle riding and the trips I would take on the bike because of the accident. That is what is depressing me. I don't have friends to do things with, and now I can't do the one thing I love -- riding my motorcycle."

"Mr. Stevens, if you aren't riding your motorcycle, what do you do?" "Jaiden, I sit around the house." "You can't do that, Mr. Steven's. Psychologically, it is the single worst thing you can do. You have to find something you can enjoy. Walking on the Erie Beach, walking the mountain trails and sitting by the waterfalls, go out to eat to simply be around people. You can make new friends doing those things. Peyton, whom you saw early, he is my spouse, and we have five baby boys. While we have to take them with us, we get out and go to our synagogue. We go to the beach, and during the week, we are still going to medical school. We became physician's assistants after helping Dr. Brannen with a difficult surgery. When he completed the surgery, my Mom, a physician, and our professor stepped into the operating room and let us know they had been watching us during the surgery. They and Dr. Brannen had made arrangements for Peyton and I to take the PA Boards. They also had made arrangements to have the tests graded immediately."

"Peyton and I passed the exam with flying colors, and the next weekend, we were making rounds here at the hospital. Working has been very healthy for us, and meeting the patients has reinforced the fact that we genuinely love and want to help people. You need to work on doing the same thing."

"Do you have a religious preference and, if you do, do you attend services anywhere?" "I am Jewish and was raised in a reform synagogue, but I haven't attended a service in years." "Peyton and I are members at Temple Anshe Hesed, and we would love to have you visit with us. It is a wonderful temple with the most gracious, loving, and accepting people you will find anywhere. You can make friends there and find events you can participate in that will get you out of the house. It is a start, and Peyton or I can take you to Saturday services until you can drive. If we have to work Saturday morning, one of our brothers can pick you up. They will take the children to the shul and pick you up in one of the other vehicles. Will you go?" "Since you care enough to ask me, I will go."

"You should be released from the hospital Wednesday, so one of us will pick you up on Saturday morning. I will need you to sign an authorization for me to tell Dale and Charley who you are and where you live so they can pick you up if Peyton or I can't. Peyton will have the authorization for you to sign in the morning when he sees you."

"I am going to write a prescription to help you sleep at night. It will be 10 mgs of Desyrel at bedtime. I will authorize the generic Trazadone, so it will be cheaper. You don't have any conditions in your history that would preclude you from taking the medication. It has to be taken with food, so the nurse will give you a sandwich and a drink before you take it. If they forget, you need to remind them."

"You can't drive while taking the medication until you know how it is going to affect you. No alcohol consumption, and be extremely careful when you stand up in case it makes you dizzy in the mornings. You don't need to fall."

"I am writing orders in your chart that you are not to get out of bed without assistance from a tech or physical therapy present." "Do you have any questions for me?" "Not right now." "Have a good evening. I will see you in the morning." "Thanks, Jaiden, for being up-front and for explaining everything." "You are welcome, Mr. Stevens, try to get some sleep tonight. The more rested and less stressed you are, the quicker you will get well."

Jaiden completed his rounds and was called to the emergency room to help with an emergency patient. The patient is a little boy who was in a wreck with his parents and has lacerations on his face. Jaiden is going to have to suture the gashes on his face and asked the nurse to get him some buffered lidocaine.

"Jaiden, I don't think we have buffered lidocaine in the ER." Jaiden told her to call the pharmacy and tell them he needed lidocaine buffered with 8.4% bicarbonate of soda to anesthetize lacerations on the face of a child. The buffered lidocaine will stop the lidocaine from stinging when it is injected.

The pharmacy very quickly got the medication to the ER, and Jaiden was able to anesthetize and suture the lacerations without the child feeling anything. He was prescribed a general antibiotic to prevent infection and was released to his parents. Jaiden told the parents to make sure the child was buckled up whenever he was in a vehicle. The parents said that it was mandatory in their cars, and they would make sure the seatbelts were checked by a mechanic and that they were working correctly.

That was the last patient Jaiden was to see, so he told the nurses he was headed for the house and to call him if they needed him for anything. The ER was fully staffed, and business had slowed to a standstill, so he was going home to cuddle with Peyton and get some much-needed sleep.

When he got home, he deactivated the burglar alarm and went into the house. As he was climbing the steps, he heard the shower running in his bedroom. Peyton heard him drive into the garage and started the shower for him. When he finished his shower, Peyton had a surprise for him.

Jaiden got out of the shower and dried off before heading to bed. When he got to the bed, Peyton had prepared himself before Jaiden got to the house, and he was lying in bed and smiling at Jaiden. As he climbed into bed, Peyton began to apply the warming lube to Jaiden and then pull Jaiden on top of him. Jaiden immediately went for the port of entry and began making slow love to his husband. When they climaxed from the pleasure Peyton provided to Jaiden, they fell asleep in each other's arms.

Peyton arose at five am the next morning, showered, dressed, and headed to the hospital after kissing Jaiden. Jaiden told him the nurses should have a consent form for Mr. Stevens sign so they could take him to the temple for services once he was released from the hospital. Peyton said he would check on that as soon as he got to the hospital. As he finished his rounds, he was called to the emergency room to assist Dr. Brannen with an emergency appendectomy. The surgery took less than an hour, and when the surgery was completed, Dr. Brannen told Peyton to do the closure. He sutured the muscle that had to be cut through and then applied a zippered closure to the incision. He would check on the patient in a couple of hours and chart his findings. Jaiden would dismiss the patient in the morning, provided everything went well overnight.

All in all, it was a good day. They each made about $4K for their weekends work. Now it was time to play with their boys and have a restful rest of their weekend. Medical school classes would resume at eight tomorrow morning. Peyton and Jaiden got up at six am the next morning, along with Charley and Dale, and got all the babies washed off, the diapers changed, and put the car seats and the feeders in the den. Charley went to the kitchen to get all the bottles warmed. After the babies were changed and cleaned, Jaiden, Peyton, and Dale got them situated in their carriers. The guys had a lot of fun with the babies this morning because the children started giggling at their Dad's antics. Ethan Robert is incredibly funny, and just like his Dad. When Jaiden picked him up to put him in his carrier, he grabbed his daddy's nose and jammed his little finger up its right side. Jaiden got a wipe and cleaned his finger while Peyton and Dale died laughing at him.

When the guys finished feeding and burping their children, Ginny and Granny walked in the door. Ginny took one look at Jaiden and asked him why his nose was bleeding. When Jaiden told her, she and Granny started laughing so hard they could barely get a breath. Ginny said, "Jaiden, I told you to trim their nails this weekend. I bet you get it done tonight!"

"Ginny, I promise their nails will get trimmed tonight. Peyton and I had a long weekend at the hospital. One of us was gone from the house all weekend with surgeries we assisted in, me having to suture a little boy's face after he was involved in an auto accident, and a patient who is so depressed he scares everybody."

"I prescribed some medicine to make him sleep at night; we will see how that works this afternoon. We won't do rounds, but we will check in with Dr. Brannen. We should be home around three today."

'OK, Jaiden, but understand this, when you get home this afternoon, and you have checked in with Dr. Brannen, you are going to spend a little time with your boys, and then I am going to make you take a nap. Understand?" "Yes, mam, Ginny. I hope we show you how much we appreciate your and Granny's help with the children. I hope they aren't stressing you out too much."

Granny said, "Jaiden, those babies are good babies. They hardly ever even whimper except when they want a diaper change, and then they really don't whimper. They are no trouble to either me or Ginny." "Good, I was hoping that was the case.

"I feel guilty, not being with them all the time." Ginny said, "Jaiden, now you understand how your Mom and Dad felt. They felt the same way you do. When you finish med school, your residencies, and get your practices set up, you will have more time to spend with your children than you realize. Now get to school and make sure you eat lunch there. If I find out you didn't, I will call both your Moms, and you know they will be here within 3 hours of my call. Do you want me to have to do that?" "No, mam, Ginny. We'll eat lunch at LECOM and I will bring you the receipt." "You don't have to do that, you are honest enough and man enough that I know you will do what you say. You are too much like your Momma not to."

Jaiden and Peyton and their brothers kissed Ginny and Granny and headed to classes.
